A pilot who thought he “should be okay” ended up dying together with his spouse in a aircraft crash.

Rhode Island Pilot Thomas Perkins, 68, and his spouse Agatha Perkins, 66, took off from New Bedford Regional Airport in New Bedford, Massachusetts, in a small aircraft sure for Kenosha, Wisconsin. The plane crashed through tree tops around 50 feet above ground, crashing into a vehicle on I-195 West before bursting into flames upon impact, about 3.6 nautical miles from the airport.

“The wreckage path continued through a wooded area, across a highway off ramp, across a portion of Interstate 195 West, and the fuselage came to rest in the median between Interstate 195 West and Interstate 195 East,” the report said.

The preliminary report assessing the site found that the aircraft’s wings sustained damage from the crash and subsequent fire, which also damaged the rudder and wings’ fuel tanks. However, the fuselage miraculously remained mostly intact.

“The main cabin door was found open when the investigators arrived; however, a witness video that was taken after the accident showed that the cabin door was closed immediately after the accident,” the NTSB report says. “Both forward seats had impact-separated from their installation points and remained within the cabin area.”

Thomas was a volunteer pilot and partner of a construction firm who lived in Middletown, R.I., with a valid pilot’s license, according to Federal Aviation Administration records obtained by The Boston Globe and his obituary. He worked for Angel Flight Northeast to provide free air transportation for patients facing medical emergencies.

He and his wife shared two children and three grandchildren, per his obituary.

“Tom was passionate about giving back and was well regarded for his generosity and charitable involvement professionally and personally,” his obituary says. “Tom and Aggie’s Perkins Family Fund supported many of the island’s charitable organizations and causes close to their hearts. Tom loved his country and was profoundly patriotic.

“Tom will be remembered for his unwavering loyalty, steadfast spirit, and deep commitment to his family, friends, and community.”

His spouse’s obituary recalled that Agatha was an energetic lady who beloved athletic actions, who her family members say can be remembered for her “infectious spirit, generous heart, sense of adventure, and the joy she brought to every day.”
